What you can expect from nutrition counseling:

60 minutes

During an initial assessment I will get to know more about you, your lifestyle, past and present medical history, any current specialized diets or allergies, along with more about you as an individual and what your food preferences are. This will allow us to work together on setting goals that will meet your nutritional needs and determine a plan of action for any nutrition education required.

30-60 minutes

Often times, frequent follow-up appointments are recommended at the start of your nutrition journey to stay on track with goals and to check in on progress made. This allows for your experience to be personalized specifically to your current lifestyle, allows for accountability, as well as laying the foundation for forming new and lasting habits. The frequency of appointments may change depending on your specific goals and desires. Follow-ups are a great way to address any new obstacles you may face during your nutrition journey. Continued support, education, and goal setting will be provided by the Registered Dietitian

À la Carte (self-pay) Appointments

We will work together to establish a starting point and initial goals to begin working towards. $92 for 60-minutes

Follow-Up (30-60-minutes) –Follow-up appointments help you stay on track with goals and check in on progress made. Additional nutrition education will be given during follow-ups as you face new challenges or life changes. This allows for your experience to be personalized specifically to your current lifestyle, allows for accountability, as well as laying the foundation for long term behavior change. $41.68 for 30-minutes

It is important to check your insurance coverage before your first appointment.

 

  1.  Call the phone number on the back of your insurance card and ask to speak with a representative about your coverage.

        • Provide Nutrition in Motion’s NPI Number: 1548437312
  1.  Ask “Does my insurance plan cover nutrition counseling?”
        • Provide the procedure codes: 97802, 97803, 97804
        • Provide diagnosis codes: Z71.3 or Z72.4
  2. Ask “How many visits per calendar year are covered?”
  3. Ask if telehealth is covered
  4. Ask “Do I have a cost-share (copay, deductible, co-insurance, etc.) for nutrition appointments?”
  5. Ask “Do I need a referral from my doctor?” Many insurance plans require a doctor’s referral before they will cover nutrition visits. Some plans may tell you one is not required, however, due to the contract between RD to Wellness, Nutrition in Motion, and the insurance company, one may still be required. Confirm with Shelby or Nutrition in Motion’s Administrative Assistants before your appointment.
